Job Description

Reporting to the Manager, Quality and Patient Safety, the Quality Improvement Specialist has a keen interest and demonstrated experience in supporting quality improvement and patient safety activities. This role will be responsible for supporting corporate initiatives to minimize harm to the patient; conducting daily surveillance on patient safety incidents, supporting root cause analysis for specific patient safety incidents and care trends while assisting managers and teams. Assisting with the patient safety incident reviews through coordinating processes of conducting the quality of care reviews, The Specialist will support with the planning and implementation of any Accreditation-related activities and support programs with their work for Accreditation along with raising awareness and understanding of accreditation and its alignment with Osler’s strategic plan. The Specialist will support in leading engagement activities with frontlines members to support knowledge translation related accreditation and patient safety incident management. The Specialist will collaborate closely with quality improvement consultants to execute quality improvement projects and initiatives providing data analysis, project management support and contributes supporting a corporate continuous performance improvement culture resulting in enhanced quality care, patient safety and a positive patient experience.

The successful candidate will actively maintain various systems databases and online applications that assist in patient safety incident management activities (e.g. the incident reporting system, the repository of critical and non-critical incidents data bases). The ideal candidate will have excellent interpersonal and communication skills, strong relationship-building skills and have an ability to prioritize and support multiple projects simultaneously.

Accountabilities:

  • Supporting multidisciplinary team meetings to investigate incidents and identify underlying causes by utilizing tools such as five whys and failure mode and effects analysis etc.
  • Complete document findings and develop action plans
  • Share RCA and gap analysis reports with relevant stakeholders and hospital leadership
  • Support process mapping by understanding the process and supporting interviews and observations with relevant stakeholders
  • Support the development of process maps in collaboration with staff at all levels
  • Support in the dissemination of recommendations for process improvements and presenting them to stakeholders
  • Provide relevant trainings on quality and patient safety principles
  • Support hospital accreditation preparation activities and survey
  • Support the development of memos, educational materials and resources for quality and patient safety
  • Support in the development of relevant presentations for hospital leadership
  • Support in the review and update as required of relevant policies and procedures to reflect current best practices
  • Conduct literature reviews to stay updated on the latest evidence-based practices
  • Connect with other organizations for environmental scans to identify and adopt best practices from other organizations
  • Support in the collection and analysis of data using relevant tools
  • Support in the development of presentations, briefing notes and reports to share findings with staff and leadership
  • Support quality improvement projects based on organizational goals by drafting project plans, timelines, and metrics for success
  • Use plan-do-study-act cycles to test and implement changes
  • Evaluate the impact of quality improvement projects and share findings

Qualifications

  • Graduate of a related health care profession
  • Member of a regulated health discipline preferred
  • Minimum of two years clinical experience to understand patient care process
  • Minimum one to two years experience of quality improvement, patient safety and process improvement
  • Proven ability to remain up-to-date with health care quality assurance trends, practices and legislation
  • Proven ability to support and coordinate activities related to quality improvement, patient safety and accreditation
  • Experience in collecting, analyzing and interpreting data for the purposes of quality improvement
  • Experience in project management an asset
  • Demonstrated understanding of quality improvement methodologies
  • Proficient in collecting, analyzing and interpreting data, excellent computer skills, experience with databases
  • Understanding of and experience with Accreditation methodology preferred
  • Experience with Canadian patient safety incident analysis framework
  • Exceptional computer skills

Additional Information

Hours: Currently days, Monday to Friday (subject to change in accordance with operational requirements)

Annual salary:
Minimum: $73,807 annually
Maximum: $92,274 annually
