Case Manager
Your role as Case Manager
Dialogue is looking for a Case Manager to take part in our mission to help people improve their wellbeing. Working closely with Sarine Moumdjian, Manager, Member Services, your role will be to ensure patients have a supportive and unforgettable experience throughout our Mental Health+ program and Employee Assistance Program. What you’ll be doing:
- Greeting new patients in the program and accompany them through their journey
- Checking in and following up on active cases, assist physicians and psychologists in their related tasks
- Analyzing cases and providing the best possible solutions to our members
- Taking a critical look at our program and generating ideas to optimize our processes and our patient’s experience ;
- Monitoring data on patient consultations and progress
- Driving usage by promoting the program to existing clients
- You will be responsible for patient safety in your member-facing role
We’d love to hear from you if you have:
- A passion for mental health, quality of care, and improving team processes and understanding
- Availability to work days, evenings, and weekends
- Availability to work a minimum of 8 hours per week, with the opportunity to work part-time (21 hours minimum) or full-time up to 40 hours per week (depending on our coverage needs)
- A strong sense of empathy, time-management, critical thinking, attention to detail, and excellent customer service
- Preferably a Bachelor’s degree in psychology or in any health-related field
- Strong computer skills
Please note that as we serve customers across Canada, bilingualism is essential for this position. You may be required to communicate in French and English.
