Lead and manage a team of community nurses delivering home care across Peel. Oversee program planning, scheduling, education, clinical workflow design, quality and risk management, client satisfaction resolution, and collaboration with operations and system partners.

Provide hands-on physiotherapy and occupational therapy assistance under a supervising therapist: assist with therapeutic exercises, mobility and ADL training, adaptive equipment instruction, home safety assessment, progress documentation, and client/caregiver education.

Provide hands-on physiotherapy and occupational therapy support under a supervising therapist: assist clients with therapeutic exercises, mobility and ADL retraining, train on adaptive equipment, monitor progress and document, educate clients and caregivers, and help maintain safe home environments.
