Teams at LOFT Community Services

Provide direct personal care and behavioural support to seniors with responsive behaviours; observe and document behaviour; help develop and implement Behavioural Support Plans; collaborate with Psychogeriatric Case Manager and multidisciplinary teams; assist with transitions, discharge planning and training for caregivers; follow health, safety and site protocols; work primarily in hospital settings with frequent travel.

Lead and manage onsite supportive housing operations and staff, ensure compliance and documentation, oversee case management and client supports, build partner relationships, contribute to program planning and evaluation, facilitate client engagement and groups, and participate in rotating after-hours on-call support for individuals with complex mental health and substance use needs.

Lead and manage supportive housing program operations across multiple sites, overseeing staff recruitment, supervision, training, and on-call coverage. Ensure client-centered service delivery, compliance, risk management, budgeting, data reporting, facilities oversight, stakeholder engagement, and quality improvement. Provide direct crisis support as needed and build community partnerships to meet client needs.
