Teams at LOFT Community Services

Provides behavioral assessments, individualized support plans, crisis intervention, counseling, education, and care coordination for seniors with dementia, mental illness, addictions, or neurological conditions. The role collaborates with families, caregivers, hospital teams, and community agencies, supports transitions from acute care, documents services, and connects clients with appropriate resources. Work occurs primarily in hospital settings with frequent travel, occasional evenings and weekends, and possible after-hours on-call responsibilities.

Provides case management and direct support to adults experiencing mental illness, addictions, physical health concerns, homelessness, and related challenges. Develops recovery plans, teaches daily living skills, coordinates medical and community services, delivers crisis intervention, advocates for clients, maintains records, and supports access to housing and financial assistance. The role includes rotating 24/7 crisis coverage, frequent regional travel, and collaboration with multidisciplinary teams.
