Leads crisis outreach operations for seniors experiencing mental health, substance use, homelessness, or social vulnerability. Oversees site compliance, daily operations, staff scheduling and performance, client support, documentation, incident reporting, program planning, quality improvement, and community partnerships. Provides coaching, advocacy, crisis support, and service navigation while participating in rotating after-hours on-call coverage.

Provides team leadership and oversees harm reduction, outreach, case management, site operations, compliance, and service quality. Supports clients experiencing homelessness, mental health challenges, substance use, HIV-related vulnerabilities, and other complex needs through advocacy, crisis intervention, referrals, and group facilitation. Coordinates staff, partnerships, reporting, program planning, evaluation, records, resources, and community outreach while promoting client independence and well-being.

Provides direct personal care and activities-of-daily-living support to older adults with dementia, mental illness, addictions, and other neurological conditions. Observes and documents behaviours, supports behavioural plans, applies crisis and intervention strategies, reports changes, mentors caregivers and staff, participates in care conferences and discharge planning, and assists with transitions between care settings. The role involves hospital and community work, multidisciplinary collaboration, frequent travel, and occasional evening or weekend hours.
