Provides strengths-based case coordination for individuals experiencing homelessness, substance use, and mental health concerns. Responsibilities include intake, assessments, individualized support and care planning, harm reduction education, referrals, crisis intervention, housing and treatment coordination, documentation, and collaboration with interdisciplinary teams and community agencies. The role involves community-based work throughout Waterloo Region, participant advocacy, motivational interviewing, discharge planning, and support across pre-treatment through recovery.

Leads organization-wide quality, safety, risk, privacy, emergency preparedness, incident management, accreditation, and program data initiatives. Oversees the Quality team and Program Data and Health Information Manager, develops integrated frameworks and reporting systems, supports emergency response and privacy compliance, and drives continuous improvement and data-informed decision-making. Collaborates with clinical and program leadership, funders, accreditors, community agencies, and the organizational leadership team to advance strategic priorities and participant outcomes.
